Employer Requirements for Optional Benefits

FDH Access 100 (no cost) Dental plan
  • Employer may not offer another dental plan to their Employees outside of the CaliforniaChoice® program
Optional Dental (plan 1000, 3000, 3500, 4000 and or 5000):
  • Employer is required to have a minimum of 70% of all eligible employees enroll in dental coverage
  • Orthodontia added to a plan requires a minimum of 5 eligible employees
  • Employees electing plan 4000 or 5000 are subject to a 12-month waiting period for major services and 24 months for Orthodontic benefits (if elected).

    Groups with 10 or more eligible employees may reduce or eliminate the waiting period by submitting proof of comparable dental coverage (to include Orthodontic coverage, if elected). Acceptable forms of proof include prior dental billing statements and a certificate booklet.
Employee Life Insurance
  • Employer is required to pay 100% of the employee's premium
  • All eligible employees must enroll in life insurance coverage
